A. Parent / Guardian Authority & Participation I confirm that I am the parent or legal guardian of the player named in this registration and have the authority to provide this consent. I give permission for the player to register for and take part in this event, including competing for any applicable prize money and ranking points. I confirm that the player's date of birth and all other information provided in this registration are true, complete and accurate.

B. Rules, Regulations & Integrity I acknowledge and agree that the player must comply with the official WNT Rules and Regulations, the Event Regulations, and all applicable integrity and conduct requirements.

C. On-Site Guardian & Emergency Assistance I understand that a responsible adult guardian must remain at the venue whenever the player is present for tournament participation, and where I have named a different on-site guardian, I authorise that person to supervise the player and act as their responsible guardian during the event. I authorise reasonable first aid or emergency medical assistance to be arranged for the player where necessary, with the responsible guardian contacted as soon as reasonably possible.

D. Safeguarding acknowledgement I confirm that I have read and understood the applicable Matchroom Multi Sport Safeguarding Policy and the requirement for a responsible adult guardian to remain on site whenever the player is present for tournament participation and to present valid government-issued photo ID for verification at event check-in.

E. Junior Privacy Notice acknowledgement I confirm that I have read and understood the Junior Participant Privacy Notice.

F. Media consent (required) I consent to identifiable photographs and video of the player being used by Bangkok Cuesports Festival and World Nineball Tour / Matchroom for event coverage, websites, social media, promotion and broadcast.
